Molina Healthcare in 2026
Molina Healthcare is a managed-care company centered on government-sponsored coverage. Its current portfolio includes Medicaid, Medicare and ACA Marketplace products rather than one national plan design. Molina reported 4,372,000 Medicaid members, 222,000 Medicare members and 267,000 Marketplace members as of September 30, 2026.
That product mix matters when evaluating the brand. A Molina Marketplace plan, a Molina Medicaid plan and a Molina Medicare plan can use different legal entities, provider networks, eligibility rules and benefits. This review uses the current ACA Marketplace footprint for the availability map while keeping Medicaid and Medicare context separate.
Latest Company Data: Third Quarter 2026
Molina's October 21, 2026 earnings release is the newest company-wide membership checkpoint available for this review. At September 30, Molina served 4,862,000 members in total: 4,372,000 in Medicaid, 222,000 in Medicare, 267,000 in Marketplace coverage and approximately 1,000 in its other segment. These numbers are people enrolled across Molina businesses, not a count of ACA plans, doctors or available counties. The 2026 Marketplace segment is far smaller than it was at December 31, 2025, when Molina reported 655,000 Marketplace members.
The same release reported approximately $10.5 billion of quarterly premium revenue and a Marketplace medical care ratio of 92.5% for the third quarter. The medical care ratio is a company financial measure of the share of premium revenue spent on medical care; it is not a clinical-quality score, customer satisfaction measure or member-level cost estimate. The company attributed part of its year-over-year premium revenue decline to lower Medicaid and Marketplace membership.

2026 ACA Marketplace Footprint
Molina says it is participating in the Marketplace in 2026 in all of its current markets except Arizona, Iowa, Massachusetts, Nebraska and New York. Molina's current Marketplace state selector and 2026 state plan materials support a 15-state ACA footprint: California, Florida, Idaho, Illinois, Kentucky, Michigan, Mississippi, Nevada, New Mexico, Ohio, South Carolina, Texas, Utah, Washington and Wisconsin.
State participation does not mean every Molina plan is available in every county or ZIP code. Service areas, provider networks and plan designs can vary within a state. Always verify the exact plan on the state Marketplace or HealthCare.gov before enrolling.

Finding Your State, County and Enrollment Path
Start with your residence, not the national carrier name. Enter your household's ZIP code in HealthCare.gov or the state exchange, select the appropriate coverage year and review every plan that actually appears for your county. A national carrier directory or Molina state selector is an indication of possible participation, not a quote or eligibility determination. If you do not see a particular Molina plan, do not assume it is sold in your locality.
For most HealthCare.gov states, open enrollment for 2027 coverage begins November 1, 2026 and runs through January 15, 2027; December 15 is generally the deadline for coverage beginning January 1. State-based exchanges can set different enrollment deadlines. If you need coverage before the next open-enrollment period, check whether a qualifying event creates a Special Enrollment Period. This timeline concerns ACA Marketplace plans, not Medicaid's separate eligibility and enrollment process.
For an apples-to-apples shortlist, save each candidate's full plan name, issuer, metal level, plan ID and network name. These distinctions become essential if several products display the Molina name but have different provider networks or deductibles.
Marketplace Plan Design and Networks
The national 2026 ACA comparison data used below places Molina in the HMO comparison set rather than the EPO or PPO comparison sets. HMO coverage typically makes the local provider network, primary-care relationship and referral rules especially important. Exact network and referral rules still depend on the specific Molina plan and state.
Molina maintains provider-search and drug-search tools for Marketplace members, and current plan documents show state-specific provider manuals, formularies and prior-authorization rules. A provider participating with one Molina product should not be assumed to participate with every Molina Marketplace, Medicaid or Medicare plan.

How to Confirm a Doctor, Hospital and Referral Rule
The safest network check has three parts. First, use Molina's provider finder for your specific 2026 plan, not a generic Molina search. Second, call the doctor's office and ask whether it accepts that exact Marketplace plan and network, rather than Medicaid or Medicare products from the same brand. Third, confirm with Molina member services using the plan ID and the provider's practice address. Ask separately about affiliated hospitals, labs, imaging facilities and any treatment already scheduled.
For HMO plans, out-of-network nonemergency care may have little or no coverage except where the plan or law requires it. Ask whether your primary-care doctor must coordinate specialist visits, whether referrals are required and which services need prior authorization. Hospital participation does not automatically establish that every clinician at that facility participates. Keep a dated record of what the directory and customer-service representatives tell you.
Network and provider participation can change during the year. Confirmation is especially important before nonurgent surgery, recurring specialist care, an expensive prescription or a move between counties. No nationwide provider total substitutes for a local directory check.
2026 ACA Cost Benchmarks
Forbes Advisor's 2026 national ACA comparison tables, which it attributes to HealthCare.gov, list Molina HMO monthly premium benchmarks of $398 at age 21, $422 at 27, $456 at 30, $512 at 40, $716 at 50 and $1,085 at 60. The same research lists Molina Bronze, Silver and Gold averages for ages 30 through 60.
These are unsubsidized research benchmarks, not Molina quotes and not a promise of local plan availability. Actual premiums can vary by ZIP code, county, age, household enrollment, tobacco use where permitted, plan design and premium-tax-credit eligibility. A lower premium also does not prove lower total annual cost because deductibles, copays, coinsurance and out-of-pocket maximums vary by plan.

Premium, Deductible and Worst-Case Yearly Cost
A low monthly Marketplace premium is only the first number in the budget. Build three scenarios for each Molina plan: a low-use year with preventive services, a normal year with your expected medications and routine visits, and a high-use year approaching the plan's in-network out-of-pocket maximum. Add the twelve monthly premiums to your expected covered cost sharing in each scenario. The out-of-pocket maximum generally does not include monthly premiums, noncovered services or certain out-of-network spending.
For example, comparing a $100 monthly premium gap means comparing $1,200 over a full year before care costs, but that hypothetical arithmetic is not a Molina quotation. Check whether you qualify for advance premium tax credits. If eligible for cost-sharing reductions, compare available Silver plans carefully: the reduction can change deductibles and copays, not merely the monthly premium.
Ask for the current Summary of Benefits and Coverage and review deductible rules for prescriptions versus medical care, office visits before the deductible, emergency services, hospital stays and specialty medicines. A lower published insurer-wide average does not guarantee lower total costs for one household.
Marketplace Member Tools and Benefits
Molina Marketplace pages describe preventive care, annual wellness visits, virtual care through Teladoc, pediatric preventive benefits and member access to provider and prescription-drug tools. Current 2026 Marketplace brochures in states such as Illinois and Washington also show 24-hour nurse advice, preventive prescription benefits and pediatric vision benefits, with adult dental or vision included only on selected plan options.
My Molina lets members manage common account tasks such as payments, ID cards, doctor searches or changes and service history. Benefits and cost sharing are plan-specific, so use the exact Summary of Benefits and Coverage, Evidence of Coverage and formulary for the plan you are considering.
Prescriptions, Virtual Care and Authorization Rules
Molina's Marketplace communications describe plan-specific virtual care, nurse advice and prescription resources. Those services can be useful, but convenience is not a substitute for confirming what the plan actually pays. For a prescription, verify the current formulary tier, generic or preferred alternatives, participating retail and mail pharmacies, any quantity limits, step therapy, prior authorization and the expected patient cost before deciding that the medication is affordable.
If you use Teladoc or another advertised virtual-care service, check the service's eligibility, whether routine and urgent virtual visits are treated differently, applicable copays and whether laboratory work or follow-up specialist appointments are covered under different provisions. A $0 preventive-care benefit does not make every visit coded at a preventive appointment free; diagnostic services can be billed differently.
Before planned procedures, ask the treating clinician and insurer who is responsible for the prior-authorization submission, what clinical documentation is required and when a decision is expected. For urgent needs, ask about expedited review. Save written denials and review the specific plan's appeal procedure rather than relying only on a telephone description.
Medicaid and Medicare Context
Medicaid remains Molina's largest business by membership. The company's September 30, 2026 segment data reported 4,372,000 Medicaid members compared with 267,000 Marketplace members and 222,000 Medicare members. Those figures describe company membership, not ACA Marketplace availability.
Molina continues to participate in Medicare during 2026, but its July 2026 Form 10-Q says the company plans to exit the Medicare Advantage-Part D product in 2027 and make its dual-eligible business the long-term strategic focus of the Medicare segment. Medicare shoppers should therefore verify 2027 continuity separately rather than assuming a 2026 Molina MAPD plan will renew unchanged.

How Molina Medicaid Eligibility Works
Molina contracts with state programs, but the state determines Medicaid eligibility and the coverage options that appear for a given applicant. Income, household composition, age, disability, pregnancy and other state-specific factors can affect eligibility. Some states enroll eligible people into managed-care plans and others provide different delivery models. The fact that Molina operates a Marketplace product in a state does not prove it also offers a Medicaid plan in the same county.
For Medicaid, check the state Medicaid agency's current eligibility rules and whether Molina is a selectable managed-care organization where you live. Families should separately verify pediatric benefits, primary-care assignment, transportation or interpreter support where included, and the specific provider network. Do not use the Marketplace premium comparison chart to estimate Medicaid member costs.
Medicare is another distinct product: Medicare Advantage, Part D and coverage for people eligible for both Medicare and Medicaid have their own enrollment, benefits and contracting rules. Molina's Medicare segment financial results do not mean that every Medicare product continues into 2027.
2027 Medicare Advantage-Part D Transition
Molina disclosed that it intends to exit its traditional Medicare Advantage Prescription Drug (MAPD) product for 2027 as the company concentrates on members eligible for both Medicare and Medicaid. The October 21, 2026 third-quarter earnings report repeated this plan and described the exit as part of its strategic shift. This is not the same as saying Molina is leaving Medicaid or all Medicare-related coverage. Dually eligible products require separate verification.
If you have a Molina MAPD plan in 2026, read any Annual Notice of Change and nonrenewal correspondence carefully, confirm when current coverage ends and review replacement options for your ZIP code and needs. Give particular attention to prescription formularies, specialists, pharmacies and total annual costs. Verify any special enrollment rights and deadlines with Medicare.gov or a State Health Insurance Assistance Program (SHIP) counselor. Do not assume that a similarly named plan has the same pharmacy network or benefits.

Ratings and Quality Context
ApexTrusted currently displays third-party editorial evidence for Molina rather than inventing its own company-wide rating without a reproducible rubric. Insure.com gives Molina a 3.72 out of 5 third-party 2026 editorial score. Forbes Advisor's 2026 health-insurance comparison rates Molina's digital experience as Poor while still showing competitive ACA premium benchmarks. Those are third-party editorial measurements, not customer-review scores or regulator ratings.
NCQA releases its 2026 Health Plan Ratings on September 15, 2026. This review does not collapse plan-level NCQA results into a single Molina brand average without a reproducible company-level aggregation. Molina also operates through multiple state health-plan entities, so this review does not assign a single current AM Best financial-strength rating to the entire Molina brand without a source that clearly supports that scope.
For complaint context, this review uses a separate same-year comparison based on NAIC MCAS 2024 individual-health complaint ratios compiled by PlainInsurer. Molina's ratio in that dataset is 1.67 versus the 1.00 industry benchmark. Because the reporting year is 2024, it is labeled as a historical comparison published in 2026 rather than a current 2026 complaint score.
Who Molina May Fit—and Who Should Compare Carefully
Molina deserves a closer look when it actually offers an affordable plan in your county, your established clinicians and necessary medicines are in the exact network and the plan's deductible and maximum exposure fit your budget. Its experience with state programs and its Marketplace member tools can be valuable for shoppers comfortable with a managed-care model and proactive network checks.
Compare especially carefully if you frequently travel, need access to a specific out-of-state specialist, have several clinicians across hospital systems or prefer broad out-of-network flexibility. An HMO plan may be a poor match despite a lower premium when essential care is outside its network. A Medicaid applicant should prioritize state eligibility and program options; a Medicare shopper should consider the announced 2027 MAPD change before assuming long-term product continuity.
The available third-party editorial scores and complaint comparisons are directional evidence, not an individualized verdict. Molina's recent companywide medical care ratios concern financial performance rather than the probability of a member getting an appointment, so they should not be presented as quality stars.
What to Verify Before Choosing Molina
Start with the exact plan available for your ZIP code. Confirm the provider directory, hospital network, formulary, referral rules, prior authorization, out-of-network rules, metal tier, deductible, copays, coinsurance, out-of-pocket maximum and financial-assistance eligibility. If you use specialists frequently, the HMO network and referral structure deserve particular attention.
Also keep product scope straight: the 15-state map below is for current 2026 ACA Marketplace participation. Molina's Medicaid and Medicare footprints are different. For Medicare, verify the 2026 plan and the announced 2027 MAPD exit before making a continuity decision.
Editorial Bottom Line and Source-Checking Checklist
Bottom line: Molina is primarily a Medicaid-focused managed-care organization with a more limited, state-specific Marketplace offering and separate Medicare products. Its published ACA price benchmarks can be attractive, but the relevant decision is whether the exact local plan covers your clinicians, medicines and expected care at an acceptable full-year cost. As of October 23, 2026, the newest reported membership is 4.372 million Medicaid, 222,000 Medicare and 267,000 Marketplace; the planned 2027 traditional MAPD exit is an important transition point.
- Confirm the product line: Medicaid, Marketplace or Medicare; do not carry coverage claims between them.
- Use the official state exchange or agency to verify current service area and enrollment eligibility.
- Compare written benefits and cost-sharing documents alongside a prescription and provider checklist.
- Verify the exact network with both Molina and your clinicians.
- For a 2026 MAPD enrollee, independently confirm any 2027 nonrenewal notices and replacement choices.
- Treat published aggregate ratings, average premiums and financial ratios as sourced context, never as a guarantee.

How this Molina Healthcare review was built
Data checked October 23, 2026. Methodology documents scope and limitations; it is not a ranking score.
- Scope
- This is a mixed-company review with the availability map and ACA cost tables specifically scoped to Molina's current 2026 Individual and Family Marketplace business; Medicaid and Medicare are discussed separately so product footprints are not conflated
- Availability
- Molina's July 2026 Form 10-Q says it participates in the Marketplace in all current markets except Arizona, Iowa, Massachusetts, Nebraska and New York; current Molina Marketplace pages and 2026 materials support the 15-state map stored here
- Marketplace strategy
- Molina reported 267,000 Marketplace members at September 30, 2026 versus 655,000 at December 31, 2025 and describes the reduction as part of its product and pricing strategy toward restoring target margins
- Costs
- Age and metal-tier premiums come from Forbes Advisor's 2026 ACA tables, attributed there to HealthCare.gov. They are unsubsidized national research benchmarks and are not personalized quotes
- Plan type
- The cited national ACA benchmark places Molina in the HMO comparison set and not the EPO/PPO sets; exact local product design and referral rules still require plan-document verification
- Benefits
- Molina official Marketplace pages and 2026 state brochures are used for preventive care, virtual care, nurse advice, digital tools, prescription support and selected dental/vision context; benefits vary by state and plan
- NCQA
- NCQA released its 2026 Health Plan Ratings on September 15, 2026. Because ratings are plan-level, this review does not infer or store a single Molina-wide 2026 average without a reproducible aggregation
- Financial strength
- No single current AM Best rating is stored for the entire Molina brand because Molina operates multiple state health-plan entities and no current source located for this review supports a universal brand-level rating
- Complaints
- The dedicated complaint comparison uses NAIC MCAS 2024 individual-health complaint ratios as compiled by PlainInsurer in July 2026. Molina is 1.67 versus the 1.00 industry benchmark in that same-year dataset. The scalar complaint_index field remains blank so this 2024 value is not silently mixed with stored benchmarks from other reporting years
- External rating
- Insure.com's 3.72/5 is displayed only as a third-party 2026 editorial score and is kept separate from customer reviews, NCQA, complaints and any internal ApexTrusted rating
- Medicare transition
- Molina's July 2026 Form 10-Q and October 21, 2026 Q3 release say the company plans to exit the Medicare Advantage-Part D product in 2027 while making dual-eligible business its long-term Medicare focus; that future change does not alter the 2026 ACA Marketplace map
- Decision guidance
- Verify the exact Summary of Benefits and Coverage, Evidence of Coverage, provider directory, formulary, referral rules, prior authorization, out-of-network terms, deductible, cost sharing, out-of-pocket maximum, subsidy eligibility and service area before enrolling

Questions & customer reviews
Frequently asked questions
Answers to common research questions about Molina Healthcare.
Is Molina Healthcare available in my state?
Carrier availability varies by state and county and can change from year to year. Check the official Marketplace and Molina Healthcare's current service-area tools using your ZIP code before relying on a carrier-level availability claim.
What plan types are usually offered?
Plan types such as HMO, EPO and PPO vary by carrier, state and service area. Use this review as a starting point, then confirm the exact network type in the current plan documents for your ZIP code.
How do I know if I qualify for a subsidy?
Premium tax credits and cost-sharing reductions depend on household income, household size and other Marketplace eligibility rules. Use the official Marketplace application or estimator for household-specific results; this review does not determine eligibility.
Can I switch carriers mid-year?
Generally you can only switch during Open Enrollment, unless you have a qualifying life event (like losing other coverage, moving, marriage, or a new baby) that opens a Special Enrollment Period.
How do I check if my doctor is in-network?
Network coverage varies by specific plan, not just by carrier. Search the carrier's directory using the exact plan and ZIP code, then confirm directly with the provider or pharmacy when possible.
What's the difference between HMO and EPO plans?
An HMO typically requires a referral to see a specialist and limits care to in-network providers. An EPO also limits care to in-network providers but usually does not require a referral.
How do I file a claim or a complaint?
Claims and member complaints are handled through the carrier's plan documents, member portal and customer-service process. For unresolved insurance complaints, your state insurance department can provide the applicable consumer-assistance route.
Is this the same as Medicare or Medicaid?
Medicare and Medicaid are separate government programs with their own eligibility rules. Some carriers, including Molina Healthcare, may offer separate Medicare or Medicaid products in addition to ACA marketplace coverage; verify program eligibility and local availability through the applicable official program.
Where do I verify current plans and enrollment options?
Use the official Marketplace or the carrier’s current plan-search tools for your ZIP code. ApexTrusted provides editorial review information; any enrollment is handled by the Marketplace, the carrier, or a separately licensed third party you choose to use.
