California health insurance market intelligence
California uses Covered California for individual and family ACA Marketplace coverage. For 2026 research, the useful statewide picture is not one premium number: California is divided into 19 rating regions, carrier participation changes by region and plan type, and final availability still resolves through the enrollment ZIP and exact product.

What actually changes a California plan decision
The state market is large, but the decision becomes manageable when the evidence is read in the right order. These are the recurring variables that materially change price, availability or practical access.
California has 19 regions and Los Angeles County alone is split between Regions 15 and 16. Statewide averages cannot replace the applicable regional table.
A carrier can participate in the state while offering HMO, PPO or EPO designs only in selected regions.
The official county rate deck publishes separate age-25 and age-40 examples across metal tiers, so one Silver example is not enough to understand the market.
Hospital snapshots are useful checkpoints, but current carrier directories and medical-group rules control actual participation.
Covered California publishes 2026 product-level quality ratings. They can inform a decision but should not be converted into an unsupported statewide carrier ranking.
Explore all 19 California rating regions
Covered California pricing evidence is organized by rating region. The locator below is deliberately schematic: it shows the statewide region system and then lets you inspect the exact county scope and rate signals without pretending the drawing is an official boundary map.

Enrollment is a weighting base
The 2026 rate announcement’s region table uses effectuated enrollment for March 2025. This page preserves that scope instead of labeling the figures as 2026 city or county enrollment.

2026 carrier and plan-type landscape
Covered California lists 11 participating individual-market carriers for 2026. The important detail is not just the carrier name: HMO, PPO and EPO participation can change by rating region.
HMO is concentrated in Regions 11 and 15–19; EPO appears across Regions 1–10 and 12–14.
PPO is listed statewide; HMO is listed in every region except Region 13.

2026 premium explorer: age × metal tier × region
The official 2026 Regional Rates by County deck publishes gross premium examples for age 25 and age 40, single status, annual household income of $31,300 and specified ZIPs. The explorer below uses the lowest published Bronze, Silver and Gold row in seven representative regional county examples; it is evidence about the published tables, not a personalized quote.
When the full Region 19 table is read across all listed products, the lowest published age-25 Silver row in the source is $373 (Sharp HMO 2 Premier Copay), not the older $385 selected-carrier floor used in an earlier project draft.
Plans marked with an asterisk in the official rate deck may not be available in every ZIP within the county. Gross premium examples should be rechecked in the current Shop & Compare tool; financial assistance can materially change the household’s net premium.
How regional rate signals changed from 2025 to 2026
Rate-change tables are useful for market context, but they are not personalized renewal estimates. The comparison below preserves the official regional averages for both plan years and adds the 2026 “shop and switch” signal without turning the data into a carrier or region ranking.
The delta is the difference in percentage points between the published 2025 and 2026 regional average rate changes. It does not measure a household’s personal premium change and it does not imply that a higher or lower region is “better.”
Keep the State context above, then compare current options using the actual ZIP, household details, provider needs and prescriptions that apply to you.
County network explorer: hospital evidence as a checkpoint

The official 2026 hospital-network overview is a proposed snapshot dated August 1, 2025. It says the list may be incomplete, can change and should be verified with the health plan. The explorer below keeps that limitation visible while making the county evidence usable.

2026 quality ratings: a separate product-level evidence layer
Covered California publishes 2026 quality ratings at the health-plan product level, using a one-to-five-star framework and component measures such as getting the right care, member care experience and plan services. This review deliberately does not transcribe star counts into a carrier leaderboard.

Covered California combines available quality results into an overall rating when enough component data exists.
The source describes more than 30 aspects of care measured against national standards.
Experience scores come from member survey responses and can be unavailable for some products.
Use the official quality page as one input alongside premium, benefits, network and drug coverage.
The quality page can include transition or historical entries such as Aetna even though Aetna exited the Covered California Marketplace for 2026. For active 2026 shopping, use the current participation data and exact Shop & Compare result together with the quality page.
Financial help and 2026 enrollment context

Gross regional premium examples are not what many households ultimately pay. At the end of 2026 Open Enrollment, Covered California reported 1,927,371 plan selections; 389,590 enrollees were in plans receiving California state subsidies, with an average $45 per month in state assistance for those enrollees.
Use the current application with household income, size and address. The enhanced federal credits that had applied through 2025 expired, so prior-year net premiums should not be carried forward automatically.
California provided 2026 state premium assistance for eligible lower-income enrollees. The current Covered California application is the eligibility source.
If the Marketplace indicates possible Medi-Cal eligibility, use the Marketplace result and California DHCS information rather than assuming Marketplace-plan eligibility.
Outside Open Enrollment, qualifying events and current verification rules determine whether an enrollment window is available.
California comparison workflow: from statewide evidence to one exact plan

The MAX State Review is deliberately broad. The final decision still becomes exact only when the user narrows the research to the enrollment address and current plan documents.
Do this before comparing prices, because the regional table and offer set depend on geography.
Use the carrier/region matrix to understand the market, then keep only products returned for the exact ZIP.
Use published examples to understand relative market shape, then rerun current Shop & Compare with household inputs.
Verify required doctors, hospitals, medical groups, prescriptions and referral rules in the exact product.
Read quality ratings with the Summary of Benefits and Coverage rather than turning one metric into a universal “best plan.”
Eligibility, net premium and provider participation can change; current official results control.
California Marketplace research questions
How many Covered California rating regions are there?
The 2026 individual-market rate materials use 19 rating regions. The applicable region depends on the enrollment address and county structure.
Why does Los Angeles County have two regions?
Covered California separates Los Angeles County into Region 15 (northeast) and Region 16 (southwest in the official 2026 rate table). The exact ZIP should be used to resolve the applicable table.
What does “shop and switch” mean in the regional rate table?
Covered California defines it as the average rate change consumers could see if they shop around and switch to the lowest-cost plan in their current metal tier. It is not a guaranteed household saving.
Are the age-25 and age-40 premiums personalized quotes?
No. They are gross published examples under the source’s single-individual, $31,300 annual-income and specified-ZIP scenarios. Use current Shop & Compare for a household-specific result.
Why is the San Diego Silver floor $373 here?
This MAX review reads the full published Region 19 product table. The age-25 Sharp HMO 2 Premier Copay row is $373; earlier project drafts used a narrower selected-carrier subset whose floor was $385.
Does a hospital in the county explorer mean it is in every plan?
No. The official hospital document itself says the proposed network snapshot may be incomplete and may change. Always verify the exact current product directory.
Are quality ratings the same as a “best carrier” ranking?
No. Covered California presents product-level overall and component quality ratings. Premiums, benefits, network fit, prescriptions and the exact product still matter.
Why do the two statewide enrollment numbers differ?
The 1,927,520 figure is the March 2025 effectuated-enrollment base used in the 2026 regional rate table. The 1,927,371 figure is total 2026 plan selections reported at the end of 2026 Open Enrollment.
Methodology, scope and direct sources
County assignments and regional signals are kept separate; the on-page map is explicitly schematic, not a boundary source.
Representative county tables use the lowest published Bronze, Silver and Gold rows for ages 25 and 40. They remain source scenarios, not quotes.
Selected hospitals are shown only as names appearing in the official county snapshot; current plan directories control.
The site does not invent an aggregate score or carrier ranking from Covered California’s star system.
Editorial scope: this is a source-backed research and comparison article for the 2026 plan year. It is not a personalized insurance recommendation, quote, eligibility determination, provider-participation guarantee or carrier ranking. Current Marketplace results and current plan documents control.
